  (electronic laboratory request form) is a simple program that allows to fill data of the request form on your computer and allows subsequent print of request form, in case you have an Internet connection it allows also its transfer to the laboratory. This program is for all co-operating doctors for free. All you need is installed program, through which the Request form can be completed directly on your computer.
